Information for Patients 

Initial Evaluations

Please plan on staying at least one hour, possibly up to two hours for this evaluation.  If you have any dentures, partials, hearing aids, and/or glasses please wear them or bring them to this appointment.  Please complete the intake paperwork (available below) and bring it to your appointment as well as your health insurance cards.  It is also beneficial for you to bring any pertinent medical records to your evaluation. 

If you should come down with COVID or have symptoms of COVID or any other illness at that time, please call and reschedule without penalty.  Masks are currently optional, however should that change, the current requirement will be posted on our door. 


Prior to receiving outpatient SLP services with us you need to be finished with any home health (nursing or therapy) services to qualify for insurance coverage. 

**Entry to the Center for Communication is from the parking lot at the back side of the building. ** 

As a new or returning patient you will need to completely fill out the appropriate intake form (child or adult see below) and bring it with you to your evaluation appointment or if your evaluation is a telehealth appointment you can fax or email the forms to us prior to your appointment.  


These are lengthy but necessary forms so please allow yourself at least 30 minutes to complete them.
